Below is the Health Home Implementation Compass/ interactive guide.

 

In the center of this page is the Health Home compass. This is surrounded by the three core levels, each of these layers can be clicked on to navigate to their corresponding level.

 

The rim of the compass represents the implementation elements which are the key directions of your Health Home journey.  

 

The second ring of the compass, houses the high-impact changes, that represent the ‘what’ of Health Home Implementation. 

 

The outer ring of the compass encompasses the practical change ideas and are the ‘how' of Health Home implementation.

To navigate through the guide, you have two options - either you can click on the rings of the compass or you can use the “Go To” navigation legend to the right. The ‘Go To’ legend is on each page of the guide and will take you to the Implementation Element and the corresponding High-Impact Changes.
