Program Supports:

Similar to the Expedition program, teams will be engaged in immersive learning experiences with other practice groups and explore 7 of the 9 highly requested Patient’s Health Home Elements, tools, and strategies to advance clinic priorities. The program consists of 3 learning sessions that combine the Implementation elements into thematic focus areas and include connections to relevant technological enablers. Throughout the program, participants will also be introduced to key tech enablers - learning how digital tools and technology can better support their practice by streamlining workflows and reduce administrative burdens.

Format: This program is for teams and can be delivered virtually, in-person or in a hybrid format over 12 months. Each session of this series is 4.5 hours. Between the sessions are 1-hour PDSA coaching calls to support teams. This program is certified by the College of Family Physicians of Canada.
