Practice Facilitation Program

This program builds capacity by training practice facilitators in quality improvement (QI) methods and coaching to introduce, test and implement high-impact changes in primary care, through 35 hours of synchronous learning supported by mentors throughout the program.

Health Home Essentials is a high-impact, accredited program where teams engage in immersive learning to explore key strategies, practical tools, and technological enablers that strengthen care delivery and drive measurable improvements.
