HIG National Practice Facilitation Network 

 

Health Innovation Group is proud to launch the 2026 National Practice Facilitator Network, a collaborative professional community designed to connect practice facilitators across Canada who share a commitment to advancing the Patient’s Medical / Health Home. 

 

We are pleased to invite primary and community care practice facilitators, quality improvement specialists, and practice support team leaders.

Join us for our first session: Feb. 17th at 11:30-12:30 MT

Advance the change process through shared learning, mentorship, and collaboration!

 

HIG National Practice Facilitation Network:

This professional community offers a structured space for shared learning, skill development and peer-to-peer connection. Participants will have the opportunity to learn from and engage with healthcare transformation leaders, including primary and community health and social care leaders, physician champions, interprofessional teams, quality improvement specialists, and researchers.  

 

The network is designed to strengthen facilitation practice while empowering primary care teams to deliver person-centred, team-based care and engage in meaningful quality improvement.

 

The goals of the network include:

  • Foster a community of practice for practice facilitators across Canada

  • Build and enrich facilitation, quality improvement, and change management skills to advance health care transformation 

  • Support consistent, high-quality implementation of Patient’s Medical / Health Home principles

  • Enhance learning by connecting like-minded individuals who are passionate about improving health and care.

Format: 

  • Hosted on Zoom 

  • 8 Network Calls

  • Occurring every two months 

  • 1 hour in length

Health Home Leaders Network: The HIG Health Home Leaders Network is for leaders to connect, share ideas and accelerate their improvement journey. The sessions take place monthly and feature specific topics followed by an open discussion amongst the group.

The goals of the session include:

  • Fostering specific leadership behaviours that inspire team members

  • Identifying potentially better practices that are effective in primary and specialty care clinic environments

  • How to identify practice champions and influencing peers in overcoming common challenges of practice renewal

Health Innovation Group Retreats: HIG also organises Canadian Leadership Retreats which provide participants with the opportunity to connect with peers face-to-face, build individual professional networks and learn about innovative approaches to practice improvement from healthcare leaders across Canada.
