Directing multiple provincial and nationally-based primary care improvement initiatives, Shaelynn leads the strategic development and delivery of dynamic virtual and in-person educational programming for Patients Medical/Health Home transformation, integrated health and social care, and practice facilitation. Through her collaboration with ministries of health and healthcare organizations, Shaelynn provides expertise in the Patient’s Medical/Health Home model, practice facilitation, change management, quality improvement, person-centred interactions, leadership development, and teams and teamwork behaviours. Shaelynn is passionate about leading health system transformation, enabling provincial health authorities and health care practices to make lasting changes that benefit patients, providers, and team.
